Looking to improve your strength, wellbeing and overall quality of life?
The Vitality Passport Program is a free program designed to support older adults in living healthier, more active lives. Participants will receive personalised support from health professionals and take part in a range of engaging group sessions focused on physical and mental wellbeing.
What's Included
3 individual consultations with a Physiotherapist, Dietitian and Occupational Therapist
1 session with a Mental Health Practitioner
9 group activity sessions

Strength and Balance
Nutrition
Cognition
Mental Health and Wellness
Who Can Participate?
The program is open to:
People aged 65 years and over
First Nations people aged 55 years and over
Cost
Free to participate, thanks to support from Murrumbidgee Primary Health Network
